Appointment Policy.

Please arrive at least 15 minutes prior to your scheduled appointment time to allow time for us to gather important medical and insurance information from you. Please bring a list of current medications and be sure to bring your insurance card to every visit. Please advise us of any changes in your name, address, phone numbers(s), insurance carrier, etc., prior to seeing the doctor. Please bring any records pertaining to your immunization history. (Please bring your shot records.) 24-hour notice of cancellations is requested.
